Department of Agriculture
For Fiscal Year (FY) 2025, USDA had a total of 159 programs that were either in Phase 1 or Phase 2 category. The total number of programs decreased by two from FY 2024. Specifically, 149 programs were categorized in Phase 1; and 10 programs were categorized in Phase 2. In FY 2024, 12 programs were categorized in Phase 2. There was a slight increase in the total number of USDA High-Priority (HP) Programs from four in FY2024 to six during FY2025. Corrective action plans (CAP) were developed and implemented to closely monitor CAP milestones for each HP Program. USDA Phase 1 programs complete risk assessments every three years. For FY2025, 32 risk assessments were completed. These risk assessments were monitored to determine if a program was susceptible to improper payments and further evaluation to avoid waste, fraud and abuse. During years when programs are not required to perform a risk assessment, they must complete a management certification to confirm that the program is at low-risk of improper payments. For FY2024, USDA's Office of Inspector General (OIG) determined the agency was noncompliant with four of the six Payment Integrity Information Act (PIIA) requirements. Issues OIG identified included: improper payment rate percentage did not meet statutory requirement; the lack to demonstrate improvement with program payment integrity; improper payment estimates were not published; and root causes for improper payments not properly identified. The agency continues to collaborate with the OIG on the necessary steps and actions to fully comply with PIIA. Some of those actions included updating measurable milestones to be accomplished in order to achieve compliance for each program; role and actions of the designated senior agency official accountable for the progress of the agency in coming into compliance; the establishment of an accountability mechanism, such as a performance agreement, with the appropriate incentives and consequences tied to the success of the senior agency official in leading the efforts of the agency to come into compliance for each program; developing and monitoring agency-level corrective actions corresponding to the identified root causes of improper payments; monitoring of the state-level corrective actions and ensuring that these corrective actions are being implemented; and enhancing corrective actions to specifically target and address the primary root causes associated with the highest improper payment percentage.

Why recovery audits are not cost effective in certain programs
Programs that determined that it was not cost effective to conduct recovery audits was based on several factors including: - processes already in place to identify, recover, and report improper payments. - low improper payment rate. - insignificant amount of improper payments would be recaptured, and the time and effort to conduct the program would not be commiserate with the funds recaptured.
Intentional monetary loss improper payments are more commonly referred to as financial fraud and are overpayments that occur on purpose. This agency reported $155.1M of confirmed fraud in this reporting cycle.
Supplemental Information
USDA used the Do Not Pay Portal data services and analytics on a continual/daily occurrence. The data offered by the Fiscal Service has been determined useful. The data received as a result of potential improper payments is used to make informed decisions regarding payment disbursement. The Department works closely with Fiscal Service to determine best search results are necessary for the various program operations. Specifically, information/statistics regarding account verification and death data sources.

Actions recommended and planned to achieve compliance
The Supplemental Nutrition Assistance Program (SNAP) was found to be non-compliant with criterion five which requires the Program to publish and meet the annual reduction target or demonstrate improvement in reducing its improper payment (IP) rate. The Office of the Inspector General (OIG) recommended that SNAP maintain a focus on continued improvement in the development and implementation of new mitigation strategies and implement appropriate corrective action that will reduce the rates year after year until the rate is below 10 precent. The OIG also recommended SNAP implement automated enhancements to better prevent and detect errors. The Food and Nutrition Service (FNS) previously identified the SNAP improper payment rate target as 11 percent or less for FY 2024 performance. The reported FY 2024 SNAP IP was 10.93 percent, a decrease of .75 percent from FY 2023. The SNAP published IP goal for FY 2025 is 10.5 percent or less and the Program is on track to meet this goal. SNAP will continue to conduct technical assistance as necessary and provide additional tools to decrease the IP rate. SNAP was found to be non-compliant with criterion six which requires reporting an IP estimate of 10 percent or less. The OIG recommended that SNAP provide information about the actions it will take to come into compliance with the Payment Integrity Information Act (PIIA) requirements. Specifically, the OIG recommended that SNAP establish measurable milestones to comply with PIIA requirements to report an improper payment rate of 10 percent or less. FNS published a SNAP improper payment rate of 10.93 percent for FY 2024, meeting the Program goal of 11 percent or less. SNAP is on track to meet the published goal of 10.5 percent for FY 2025 performance, and 10 percent for FY 2026. To achieve these targets, FNS will continue to conduct training for State agencies on an ongoing basis and will provide additional tools to conduct root cause analysis and identify trends in error rate data. Additionally, FNS awarded approximately $33 million in earnings verification support grants to 38 SNAP State agencies for the purpose of supporting States’ use of electronic earned income verification services, recognizing that inaccurate income is a common cause of SNAP payment errors. In FY 2025, FNS facilitated two meetings focused on assisting States in determining their root causes of errors and corrective actions to address them. In FY 2026, FNS will launch a new, modernized payment accuracy data management system, which will include enhanced data analytics features to assist State agencies in better utilizing their data for root cause analysis and corrective action planning. The Child and Adult Care Food Program (CACFP) Family Day Care Home (FDCH) component was found to be non-compliant with criterion three, which requires the FNS to publish improper payment estimates for all programs and activities identified as susceptible to improper payments under its risk assessment. While in FY 2024, FNS published an improper payment rate for the CACFP FDCH component, it did not include meal counting errors in that calculation, which the OIG determined meant the rate was incomplete. To that end, FNS is currently undertaking an initiative to develop an improved estimate for meal counting errors within the CACFP FDCH component. Despite FNS conducting multiple studies over the years to identify a reliable method for measuring these errors, a cost-effective solution to the agency’s reporting challenge has yet to be achieved. While continuing this longer-term initiative, FNS has reported an interim rate of meal counting error in CACFP FDCHs for FY 2025, using what we consider to be an upper bound on the estimate. With the inclusion of this interim rate, FNS believes the agency is now in compliance with criterion three. The Summer Food Service Program (SFSP) was found to be non-compliant with criterion three which requires FNS to publish improper payment estimates for all programs and activities identified as susceptible to improper payments under its risk assessment. The non-compliance was the result of having not yet implemented a method to assess and report a rate. The OIG recommended that the agency continue work on the planned study to develop an error rate for SFSP, which is described in FNS’s Sampling and Estimation Methodology Plan (S&EMP). FNS also agreed to report measurable milestones to be accomplished in order to achieve compliance. To that end, FNS has been working to establish improper payment rates for SFSP in accordance with PIIA via an improper payment study. This study began during the summer of 2024, when FNS staff conducted data collection efforts for the study and went to approximately 70 SFSP sites across 7 states. In these unannounced site visits, FNS staff observed meal service to compare FNS staff-collected meal counts with those recorded by the site staff and subsequently reported to the sponsor for reimbursement. Throughout FY 2025, FNS prepared mechanisms to collect records from states in order to estimate an error rate from site observation data. During FY 2026, FNS will report an update to the S&EMP to describe the data collected and detail the methods used for estimating an error rate. FNS will then report this error rate for FY 2025 in its FY 2026 Agency Financial Report, bringing the program into compliance. FSA had 5 programs – Agriculture Risk Coverage Price Loss Coverage (ARC/PLC), Coronavirus Food Assistance Program (CFAP), the Livestock Forage Disaster Program (LFP), Noninsured Crop Disaster Assistance Program (NAP) and the Emergency Conservation Program (ECP) that were deemed non-compliant in the FY 2024 OIG Compliance Audit. Non-compliance letters and plans were sent to OMB, GAO and Congress for programs that were 3rd and 4th year non-compliant. The following summarizes the non-compliance issues noted by OIG, actions taken and planned by FSA to bring the programs into compliance, and OIG’s recommendations. FSA was found to be non-compliant due to the following issues: • CFAP - Corrective action plans were not published/prepared under section 3352(d); • CFAP, ARC/PLC, NAP and ECP - Did not demonstrate improvements to payment integrity or did not reach a tolerable improper payment or unknown payment rate; • LFP, ARC/PLC, NAP and ECP - Reported improper payment rate was over 10% CFAP was deemed non-compliant because the program did not properly identify and publish the root causes of the improper payments within the FY24 USDA AFR and/or PaymentAccuracy.gov. Specifically, the program only reported the related Cause Category (i.e., failure to access data/information). The CFAP program was no longer an active program when corrective actions were developed. Therefore, specific root causes were not included. However, FSA did consider CFAP related findings as part of the agency’s internal control roadmap to improve overall processes in the future. For ARC/PLC, LFP, NAP and ECP, to correct deficiencies and strengthen overall program integrity, FSA implemented a comprehensive set of corrective actions across (4) major areas – Policy, Software Enhancements, Training, and Internal Control Processes. Policy Highlights of policy related actions include: • An acreage reporting task force was formed in the fall of 2023 to discuss options for improving late-filed acreage report policy. 2-CP late filed acreage reporting policy was amended to include exceptions that waived the requirement to validate existence and deposition of the crop. • FSA annually issues an internal directive (Notice IA-13 – FY2024 National Payment Integrity Reviews, issued January 13, 2025) that outlines and reviews common and program specific error findings and the associated policy reference for review. • Additionally, FSA’s Administrator issued an internal compliance memo to all FSA employees on April18, 2025, which reinforced the importance of program integrity, set expectations for staff to proactively address internal and external compliance findings and corrective actions and announced planned and reiterated existing compliance activities to strengthen FSA programs through risk management and accountability. Software Enhancements Highlights of software enhancements include: • A software enhancement was developed which will be implemented with the 2026 crop year to alert County Offices (COF) employees when a producer is late filing an acreage report for a specific crop. CARS will read the acreage reporting date in the National Crop Table (NCT), and a pop-up window will appear asking if an approved exception for late filing exists. If not, late filed procedures will apply according to policy. • LFP software enhancement to interface with the National Crop table to automatically populate the carrying capacity for the specific forage used in the CCC-853 LFP application to ensure proper payment calculation was completed in February 2025. • FSA developed new ECP software and dashboard to process and monitor cost-share applications and payments more efficiently aimed at reducing administrative errors beginning with disasters occurring after October 1, 2023. The new ECP software and dashboard was released into production in January 2024. In FY25, FSA will be able to analyze the payment integrity of payments audit of payments issued in FY24 to determine if additional software provisioning is necessary to mitigate the administrative errors from occurring. • In FY24, FSA developed ECP scenarios to recognize regional differences in how projects are installed, taking into consideration climate, soil types, costs, and state standards. This ensures payments more accurately reflect local costs for practice installation. • A new ECP software and dashboard, was developed to process and monitor ECP cost share applications and payments more accurately, efficiently and reduce administrative errors. Training Highlights of training sessions include: • Bi-weekly calls are held with State office specialists providing specific policy training and answering questions. All calls are recorded and posted on the Deputy Administrator for Farm Program’s (DAFP) SharePoint page. • A series of acreage reporting training courses were developed in AgLearn for employees. New employees are required to take these courses, while all current employees will have access to them. • DAFO required training on Delegation of Authority to County Offices for March 2025, with a completion window of thirty days. This will be in addition to the information that is provided to County Office Committees. • FSA conducted targeted training sessions for all state and county offices to ensure all field office staff are aware of updated policy requirements and deficiencies identified in the PIIA audit. In addition, FSA headquarter staff provided training for state and county offices immediately following a qualifying disaster event on an as-needed basis to expedite implementation and ensure staff are adequately trained to administer ECP. Internal Control Processes • In March 2025, the Internal Controls PIIA Findings Dashboard was published and includes all PIIA tested programs and findings from fiscal years 2021-2024. This dashboard adds another data source for FSA, providing the ability to identify targeted training needs across States and County Offices based on PIIA findings. • FSA will continue to utilize the Internal Review and Documentation Tracking System (IRDTS) to conduct and monitor internal reviews of selected programs. Building on its established use, IRDTS serves as the centralized platform to standardize assignment and complete internal reviews. The system documents and tracks compliance findings identified during internal review cycles and provides dashboards and reports to ensure timely oversight of internal reviews. IRDTS strengthens accountability and reenforces FSA’s commitment to systematically addressing program compliance issues, reducing improper payments, and supporting long-term PIIA compliance. • FSA has been working on data mining for high-risk programs and has made progress towards providing tools for the National Office Program Managers and Field Offices used to assist with collectively identifying common findings. Through the use of the Department of Agriculture’s Enterprise Data & Analytics Reporting Tool (EDAPT), the Agency has been automating the flow of FSA program data into a secure Amazon environment that has allowed data mining tools as well as the visualization of the results of those findings and analysis (such as dashboards) to be built to help identify situations that may lead to an improper payment and help reduce them. • FSA has various data tracking tools available for payment integrity program-level corrective actions and findings at the program and state level: • USDA’s Office of Chief Financial Officer (OCFO) has the CAP tracker that monitors quarterly updates to the program-level corrective action plans. • The FSA Improper Payment Corrective Actions app will track individual findings through corrective action completion. • A PIIA Findings dashboard was created by FSA to track where payment errors are occurring, common findings and identify training needs. Based on the non-compliance issues noted, OIG recommended that FSA work with the USDA Office of the Chief Financial Officer (OCFO) in taking the following actions to ensure compliance with the Payment Integrity Information Act of 2019: • USDA and FSA update its policies and procedures over the reporting of expiring programs to identify and report root causes of improper payments in order to formulate corrective actions that are focused on the core issues that are under the control of the agency • Improve processes and efforts to evaluate the effectiveness of corrective actions to assist in the reduction of the improper payments estimate rate below the 10% threshold • Develop and monitor agency-level corrective actions corresponding to the identified root causes of improper payments • Conduct ongoing monitoring of the state-level corrective actions and ensure that these corrective actions are being implemented • Continue working with states to identify state-specific corrective action plans and ensure that these corrective actions are being implemented • Enhance corrective actions to specifically target and address the primary root causes associated with the highest improper payment percentage In coordination with USDA’s Office of the Chief Financial Officer and the Performance, Accountability and Risk Division, FSA remains fully committed to strengthening program integrity and reducing improper payments. The corrective actions above such as policy updates, enhanced internal controls, targeted training, and software enhancements will be used to drive measurable reductions in future error rates. These efforts reflect FSA’s ongoing dedication to improving program stewardship and ensuring FSA’s programs continue to serve producers with accountability and transparency.
