The U.S. Department of Health and Human Services (HHS) continues to strengthen payment integrity, reduce improper payments, and protect taxpayer resources. In alignment with the Payment Integrity Information Act of 2019 (PIIA), HHS conducts annual risk assessments, measures improper and unknown payments for risk-susceptible programs, and implements corrective actions to address documented root causes.
In FY 2025, HHS completed 97 risk assessments for programs with annual outlays over $10 million and identified no programs as newly susceptible to significant improper payments. Programs not scheduled for assessment were monitored through ongoing oversight and internal controls reviews. HHS published improper and unknown payment estimates for 10 risk-susceptible programs, including Medicare, Medicaid, CHIP, APTC, Foster Care, CCDF, and Head Start.
Across these programs, HHS estimated $98.34 billion in improper and unknown payments in the 2025 reporting year. Changes in error rates varied by program and reflected factors such as increased eligibility redeterminations and provider enrollment reviews following the unwinding of COVID-19 flexibilities. The most common root causes were insufficient documentation, eligibility determination errors, and provider screening or enrollment non-compliance.
HHS continued to advance corrective actions designed to prevent and reduce improper payments. Key efforts included enhanced eligibility verification through federal data sources, expanded use of the Do Not Pay system, more rigorous provider screening and oversight, greater use of analytic tools to detect high-risk claims, and targeted training and technical assistance for states and providers. HHS also conducted audits and policy reviews to strengthen compliance in Medicare, Medicaid, and CHIP, and expanded oversight activities in APTC, Foster Care, CCDF, and Head Start.
HHS was not fully compliant with PIIA for FY 2025. The Office of Inspector General (OIG) identified several areas of non-compliance, including programs with reported improper payment rates above 10 percent, incomplete reporting by certain risk-susceptible programs, instances where risk assessments were not completed within the required cycle, and delays in select recovery audit activities. HHS is addressing these issues through expanded corrective action plans, improved internal controls and documentation processes, increased coordination with states and contractors, and enhanced monitoring to ensure timely completion of required activities.
HHS remains committed to improving the accuracy of program payments and strengthening payment integrity across the Department. As corrective actions mature and operational improvements continue, HHS expects to demonstrate progress in future reporting cycles.

Conditions giving rise to improper payments identified in recovery audits, how those conditions are being resolved, and the methods used to recover those payments
Improper payments identified through recovery audits typically arise from insufficient documentation, inaccurate billing, incorrect eligibility determinations, or errors in coordinating benefits with other insurers. HHS resolves these conditions through targeted corrective actions such as enhanced provider training, updates to billing and enrollment guidance, and improved data-sharing to strengthen the accuracy of eligibility and coverage information. To recover overpayments, HHS and its contractors employ several methods, including issuing demand letters, offsetting future payments, requiring direct refunds, and using Treasury’s debt collection and offset processes when debts remain unresolved. Medicare Fee-for-Service and Medicare Advantage overpayments are recovered through recovery auditors and Risk Adjustment Data Validation audits, while Medicare Part D recoups payments from plan sponsors if documentation does not support drug event submissions. Medicaid, CHIP, and ACF programs also recover disallowed costs through federal–state reconciliation processes, direct recoupments, and corrective actions informed by Payment Error Rate Measurement or program-specific reviews, ensuring improper payments are resolved and program controls continue to improve.
Why recovery audits are not cost effective in certain programs
HHS determines that recovery audits are not cost-effective for certain programs due to low recoverable amounts, existing controls that already mitigate improper payments, or the potential for adverse effects on vulnerable populations. In ACF programs, conducting recovery audits could reduce resources available to support low-income families and children, and the administrative burden of audits would outweigh the limited recoverable funds. For the Advance Premium Tax Credit (APTC), established processes—such as annual tax reconciliation and data matching with IRS and DHS—already recapture overpayments and reduce the need for additional audit activities. In Medicaid and CHIP, states operate Recovery Audit Contractor (RAC) programs and independently assess whether recovery audits are cost-effective based on program size, administrative capacity, and expected recoveries. Across these programs, HHS concludes that the cost and operational impact of recovery audits would exceed the benefits, making them an inefficient use of federal resources.

HHS continues to use the Do Not Pay Working System to strengthen pre- and post-payment controls and prevent improper payments. In FY 2025, HHS screened 1.2 million payments totaling $959.9 billion against death record databases, identifying 51 potential improper payments, of which on 2 were confirmed—likely due to upstream use of DNP tools by program offices during pre-award and pre-payment processes. CMS also screened over 1 billion Medicare and Medicaid payments through Treasury's DNP portal, stopping 333,207 payments and preventing an estimated $2.8 billion in improper payments. Collectively, these activities demonstrate that the Do Not Pay Working System continues to reduce improper payments and strengthen HHS’s overall payment integrity efforts.

The OIG concluded that HHS was non-compliant with several PIIA requirements in FY 2024, including the requirement to conduct risk assessments for all programs with annual outlays over $10 million at least once every three years. In total, six programs—Medicare FFS, APTC, UIP, TANF, Foster Care, and Head Start—were found non-compliant with one or more criteria. For Medicare FFS, OIG determined the program had not demonstrated sufficient improvement in its improper payment rate and recommended strengthening corrective actions; HHS is expanding provider outreach and refining corrective action plans. For APTC, OIG found the program non-compliant because HHS has not yet produced a complete estimate for State-Based Exchanges; HHS is advancing methodology development and preparing states through the Improper Payment Pre-Testing and Assessment initiative. For UIP, OIG found the sampling methodology insufficient, but HHS will resolve this finding because the program has been discontinued.
For TANF, statutory limits prevented HHS from collecting data needed to produce an estimate, and OIG recommended pursuing legislative changes; HHS continues coordinating with OMB and Congress to secure the necessary authority. For Foster Care, OIG concluded that the FY 2024 estimate was not statistically valid due to incomplete state review coverage; HHS is implementing a structured multi-year review schedule to achieve full state rotation by FY 2027. For Head Start, OIG found the improper payment rate exceeded 10 percent and recommended stronger documentation controls; HHS is enhancing monitoring, providing targeted technical assistance, and issuing disallowances where documentation is insufficient. HHS continues to strengthen its payment integrity efforts by expanding the use of data analytics, improving coordination with federal and state partners, and modernizing systems to prevent improper payments before they occur. The Department is enhancing eligibility and provider screening processes, increasing the use of automated pre-payment checks such as the Do Not Pay Working System, and investing in improved training and technical assistance for states, providers, and grant recipients. HHS is also advancing new tools and pilots—such as expanded predictive analytics and real-time monitoring initiatives—to identify risks earlier and support more effective oversight. These efforts reflect HHS’s ongoing commitment to safeguarding taxpayer resources while ensuring timely access to essential health and human services programs.
